Fixed the issue that the glossary is not parsed and linked.

1. replaced mysql_real_escape_string() with $addslashes()
2. sanitized integer fields in lib/at_form_util.php -> at_get_field_value()

Always use default theme as the fundamental theme to search for missing template, even for mobile devices. The old way was to use the mobile theme as the fundamental theme for all customized mobile themes. However, mobile theme does not contain all the templates yet.
